INTRODUCTION TO HYPERCOAGULABILITY

Hypercoagulability or hypercoagulable state is also termed as thrombophilia which is an abnormally increased disposition to develop blood clots, i.e. thrombosis. It occurs due to the presence of several predisposing elements that can either be acquired or inherited. Normally clotting is a good process that helps in healing injuries and lacerations but when this clotting becomes unnecessarily too much, it is known as hypercoagulability. In this state, the blood clots are developed within the blood vessel and can develop thromboembolism like pulmonary embolism or deep vein thrombosis. These deep vein thrombosis can travel to different organs like heart, brain, kidneys and liver hampering the blood supply to them and resulting in infarction.

Hypercoagulability is of two types, i.e. first where the causative factors are inherited that means error in natural mechanism of anticoagulation and another is secondary hypercoagulability in which the causing agents are acquired like smoking, cancer, obesity, surgery, pregnancy, autoimmune or inflammatory disorders, oral contraceptives and prolonged immobilization.

In Ayurveda the hypercoagulability or thrombosis is correlated with Vatharaktham and Siragrandhi which is known to be caused due to the imbalance of Pitta, Vata dosha and Rakta dhatu. As per Ayurveda, restoring the balance of these doshas and dhatu can restore health.

CAUSES OF HYPERCOAGULABILITY

Hypercoagulability is caused due to the interaction of platelets, blood vessels and coagulation factors. These coagulation factors in inactive forms keep on circulating in the blood and when there is any injury coagulation factors VII binds to the exposed blood vessel wall’s cells resulting in activation of coagulation cascade. Further there occurs activation of other clotting factors as well that eventually causes formation of factor III (thrombin). At last this thrombin get converted into fibrin that is the main factor required for forming connected tightly mesh which maintains a platelet plug. There are different anticoagulant mechanism which prevents ingrown this clot very big or its unnecessary formation. Hence, any imbalance in antithrombogenic and thrombogenic mechanism increases the risk of developing hypercoagulability.

DIAGNOSIS OF HYPERCOAGULABILITY

Usually the hypercoagulability comes in notice of the patient and doctors after an event of thrombosis. Certain blood tests will confirm the diagnosis and further if the condition is acquired it will be cleared in clinical history and if it is genetic certain other tests will be required to confirm the genetic defects. Screening is only done in people who have high risk of inherited clotting disorder like family history of thromembolism, history of frequent miscarriages, or thrombosis in usual blood tests or at young age.

Certain anticoagulant therapy is given including drugs like warfarin, low molecular weight heparin or rivaroxaban etc. Surgery can also be done in severe cases or thrombolytic medications are prescribed. People with several hypercoagulable risks are also recommended anticoagulant medications. Also patients are advised to remove certain risk factors like smoking, oral contraceptive pills or avoiding too long immobilization.
