Vasculitic Neuropathy and Its Ayurvedic Management
Abstract
There are a lot of medical conditions. Some of them mimic each other that it is so difficult to differentiate between them. Many times a disease remains undiagnosed for long until it gets worse and chronic. Vasculitic neuropathy is one of them. Vasculitis is a type of illness in which inflammation of the blood vessels occurs. The inflammation of the blood vessels may occlude the pathway of the blood supply to the organs resulting in subsequent ischemia. When the blood vessels which supply the peripheral nerves get inflamed, it results in vasculitic neuropathy. In the following text, we shall look into a detail of vasculitic neuropathy and its management. Let’s discuss!!!
Introduction
The inflammation of the blood vessels which supplies to the peripheral nerves is known as vasculitic neuropathy. The condition can manifest with a systemic issue or can manifest as a single organ problem (nonsystemic vasculitic neuropathy) that only affects the peripheral nervous system. It can involve the kidneys, lungs, skin and other organs. In Ayurveda, the condition can be correlated with vata and pitta vitiation. As the inflammation and neurological symptoms are caused by pitta and vata dosha vitiation respectively. So vata and pitta imbalance can be ideally correlated with Vasculitic neuropathy.
Causes
- Idiopathic – When no known cause can be diagnosed.
- Autoimmune – when the immune system attacks on itself causing inflammation of the blood vessels supplying the peripheral nervous system.
- Allergic – when the allergic symptoms due various agents occur results in inflammation of the blood vessels.
- Infections – infections like HIV, Hepatitis B , Hepatitis C and others can lead to vasculitic neuropathy.
Types of Vasculitic Neuropathy
- Primary systemic vasculitic neuropathy
- Secondary systemic vasculitic neuropathy
- Non-systemic vasculitic neuropathy
1. Primary Systemic Vasculitic Neuropathy
In primary vasculitic neuropathy, multiorgan disease spreads to the peripheral nervous system.
Examples include IgA vasculitis, polyarteritis nodosa, eosinophilic granulomatosis with polyangiitis etc.
2. Vasculitic Neuropathy Secondary to Other Disease
Patients with prior disease of the non-vasculitic system can gradually develop vasculitic neuropathy.
Examples include rheumatoid arthritis, systemic lupus erythematosus, hepatitis B, human immunodeficiency virus, malignancy etc.
3. Non-systemic Vasculitic Neuropathy
When no history or illness of any system is found and the symptoms of vasculitic neuropathy are established then the diagnosis of non-systemic vasculitic neuropathy is achieved. A biopsy of the nerve is required to confirm the diagnosis of non-systemic vasculitic neuropathy.
Symptoms
- Symptoms depend on the location and type of the nerve involved. Oftentimes, both sensory and motor nerves are involved and affected.
- Sensation of paresthesias
- Numbness
- Weakness
- Pain
- Asymmetrical affect on the limb
Diagnosis
- Medical history is important
- Electromyography
- Nerve conduction studies
- Skin biopsies
- Nerve biopsies
- Muscle biopsies
- Chest X-Ray
- CT Scan and MRI
Treatment
- Aim of the treatment is to control blood vessels’ inflammation.
- Immunomodulators
- Corticosteroids (prednisone, methylprednisolone etc)
- Intravenous immune globulins (IVIG)
- Plasmapheresis
- Anti-seizures medications
- Analgesics
- Antidepressants
