Abstract

The peritoneum is the word used for the membrane which lines the abdominal cavity and retro means behind it. This clears the meaning of Retroperitoneal fibrosis which means fibrosis behind the peritoneal cavity of the abdomen. This fibrosis is the result of a rare inflammatory disorder which eventually causes the formation of abnormal fiber-like tissue. This fibrous growth does not remain limited to the abdomen but spread up to the ureters as well which carries urine from the kidneys to the bladder. Gradually there occurs blockage or compression of the ureters by excessive growth of fibrous tissue. Retroperitoneal fibrosis can result in kidney damage. Retroperitoneal Fibrosis

Introduction

Ormond’s disease is the other name given to Retroperitoneal fibrosis which is a rare condition. The exact location of the fibrosis development starts from the area between the stomach and intestine that is called the retroperitoneal area. In this, connective tissues start to grow excessively resulting in the formation of an abnormal mass.  This mass compresses or blocks the ureters, one or maybe both. Hence, toxins start to accumulate in the blood due to backing up of urine in ureters. Eventually, kidneys start to fail. However, the prognosis of Retroperitoneal fibrosis can be good if diagnosed early and proper treatment is given but with the delay of treatment kidney failure occurs. All this starts due to the inflammation and fibrosis of the abdominal aorta which is one of the large abdominal arteries. The abdominal aorta supplies the blood from the heart to the region below the kidneys. With the advancement of the disease, arteries supplying to the kidneys and legs also get affected resulting in pain and swelling in the legs and decreased functions of the kidneys.

Signs and Symptoms

Signs and Symptoms appear due to the reduction of blood flow from the abdominal aorta to the lower parts of the body and eventually kidney failure. Some of the major observed symptoms are:

  • Pain in the back and abdomen which is dull in character
  • Pain between the back and upper abdomen on one side
  • Pain in legs
  • Legs, one or both undergoes discoloration
  • Swelling on legs, especially on one leg
  • Severe pain in the abdomen with bleeding
  • Weight loss
  • Fever
  • Nausea
  • Loss of appetite
  • Low hemoglobin
  • Yellow discoloration of eyes and skin
  • Reduction in urine
  • Cloudiness of thoughts

Causes

The exact cause behind Retroperitoneal Fibrosis is not known and most of the cases are idiopathic. However, it is found to be associated with some of the factors:

  • Tumors in the abdomen or pelvic region
  • Tuberculosis.
  • Trauma to the pelvis or abdomen in recent times.
  • Bacterial infection, especially actinomycosis.
  • Recent surgery on the pelvis or abdomen.
  • Certain cancer treatment.
  • Some medicines are used to treat hypertension and migraine.

Diagnosis

Retroperitoneal fibrosis is usually diagnosed by MRI and CT scan and some other tests used in the procedure are kidney function test, blood tests for inflammation marker and anemia, Intravenous pyelogram which is an X-ray of the ureter and kidneys that reveals the blockage due to fibrosis, Ultrasound of kidneys and Biopsy to rule out cancerous cells.

Treatment

Early diagnosis is the key to better prognosis and preventing surgery. Treatment of Retroperitoneal fibrosis is completely based on the location of the fibrous tissue and the severity of the disease. General treatment includes draining procedures for the ureters to clear the obstruction through a drainage tube or stent. In advanced cases, surgery is required.

1. Boswellia Curcumin

Boswellia curcumin herbal capsules are prepared from standardized extracts of Sallaki (Boswellia serrata) and active compound of turmeric (Curcuma longa), i.e. curcumin.
