Abstract

The chlamydiae comprise a small group of gram-negative microorganisms that are obligate intracellular parasites lying intermediate between bacteria and viruses. Formerly classified as viruses, it is now apparent that they are more closely related to bacteria and are susceptible to antibiotics. The infective particles contain bacterial-type cell walls, RNA and DNA, replicates by fission, and possess the metabolic enzyme pathways necessary for survival. However, they are unable to synthesize their own high energy phosphates, as for example, ATP. Today, we will read about chlamydia trachomatis in brief.

Introduction

It is a common occurring sexually transmitted disease caused by bacteria. It affects mostly young women, but it can occur in both men and women and in all age groups It can be caused by intercourse, oral, and anal sex. These diseases may be localized to affected areas or may be disseminated. These infections usually present in the form of discharge or ulcers at the site of exposure. The organisms affect the columnar and transitional epithelium of the genitourinary tract. The lesion is limited superficially. As there is no deeper penetration, the pathological changes to produce symptoms may not be apparent. The infection is mostly localized in the urethra, bartholin’s gland and cervix. It can ascend upstairs like gonococcal infection to produce acute PID. Too often, it is associated with gonococcal infection.

The replicative cycle of chlamydiae is quite distinctive and comprises three stages

  1. Attachment and penetration into the host cell of the elementary body
  2. Development of a large “initial” body
  3. Division of the “initial” body into elementary bodies.

Causes

  • Chlamydia trachomatis is caused by bacteria known as chlamydia trachomatis bacterium which is spread during vaginal intercourse, oral and anal sex.
  • It spread through infected pregnant women to her child during delivery.
  • It can be spread through blood transfusion.

Symptoms

Clinical features are non specific and asymptomatic in most cases. Following are the symptoms seen in some cases:-

  • Vaginal discharge
  • Abdominal pain
  • Pain during urination
  • Discharge from penis
  • Pain via sexual intercourse in females
  • Pain in testes
  • Vaginal bleeding after sex

In some cases findings include

  • Mucopurulent cervical discharge
  • Cervical edema
  • Cervical friability

Risk Factors

  • Sexual intercourse without contraceptive measures
  • Multiple sex partners
  • Not taking precautions against sexually transmitted disease

Complication

  • Pelvic inflammatory disease
  • Prostate gland infection
  • Ectopic pregnancy
  • Infertility
  • Infections in newborns via mother
  • Epididymitis

Prevention

  • Use contraceptives via intercourse
  • Limit the number of sexual partners
  • Get regular screening
  • Avoid douching

Diagnosis

  • In uncomplicated cases, the materials are to be collected from the urethra and cervix.
  • Chlamydia can be demonstrated in tissue culture
  • Antigen can be detected by ELISA technique
  • Serology to detect micro immunofluorescence antibodies can be useful in the diagnosis of PID but not in infection limited to the lower genital tract.
  • Nucleic acid amplification test is performed although antigen can also be detected by direct immunofluorescence antigen test.
