These lesions are known as intraductal papillomas, which are non-cancerous tumours that grow within the milk ducts. They are made up of fibrous tissues and blood vessels called fibrovascular tissues. Introduction
Intraductal papilloma is a benign (non-cancerous) and painless tumour found within the milk ducts. This is the result of abnormal growth of ductal epithelial cells (these are the cells that cover the surface of the body from inside to outside). It is found centrally and posteriorly to the nipples. There is a lumpy lesion under the breasts known as a papilloma. These lesions may be palpable, but when they are small, they are not palpable. This also manifests as a serous, watery discharge from the nipples. The discharge is usually spontaneous. Intraductal papillomas are generally less than 1cm in diameter and 3 to 4mm in size. These are pink or black in color. Women of all age groups can develop this tumour but it is most common in the age group of 35 to 55 years. There are no such risk factors for this tumour. Moreover, the chances of recurrence are lower.
Causes
Both men and women can get these lesions, but they are more common in females in the over-40 age group. Well, the exact cause of intraductal papilloma is unknown; there is only proliferation in the epithelial cells. Some risks exist there, due to which intraductal papillomas are affected. They are as follows:
- Family history of intraductal papilloma
- Use of hormonal contraceptives.
- Hormonal replacement therapy
- Oestrogen exposure
Symptoms
- Unusual watery discharge, sometimes bloody, from the nipples is the main symptom of intraductal papilloma.
- A small lump behind the nipples.
- Usually they are not painful, but some women do have discomfort or pain around the area.
Management
In modern times, there are only surgical procedures mentioned as per management. It involves the excision (removal) of the breast lumps. But sometimes it causes severe infections. This increases the chances of recurrence. But in ayurveda, it is manageable with medications explained by different acharyas in their particular textbooks. Surgery is not the ultimate treatment for intraductal papilloma.
Ayurvedic View
In Ayurveda, there are different types of granthi explained by different acharyas in their literature. Intraductal papilloma is correlated with vataj granthi. Different acharyas have explained the different meanings of the word granthi. Some of them gave the meaning of swelling or protuberance; some called it nodular or glandular swelling. Granthi develops due to the vitiation of doshas and dushyas, followed by their aggregation in one place. Generally, granthis do not cause the death of the patient, but if avoided, they can cause complications.
Definition
As discussed above, granthi means a nodular swelling, but when it is associated with vata, the definition changes a little as vata dosha has some specific properties. Granthi, which develops due to the vitiated vata dosha in any part of the body, has the specific features of vata like dryness and pain (different types of pain are experienced). Different acharyas give their own explanations regarding the features of pain in vataj granthi. Some say that it has a stretching, perforating, and piercing type of pain. Some acharyas mentioned that the pain is pricking, churning, and throwing. This granthi has a shaking or trembling type of pain that ruptures if it is pierced, cut, or perforated. Even the color of vataj granthi is mentioned by many of the acharyas. It is mostly black in colour, not very soft, freely mobile, spreads, and increases or decreases in size. On rupture, there is a discharge of fresh blood from the nipples. Sometimes, the discharge is clean.
Samprapti
With the vitiation of vata dosha, vitiated mansa (muscles), rakta (blood), and meda (fatty tissue) mixed with kapha to produce rounded, projected, nodular, and hard swelling. Aggravated vatadi doshas, mansa, and rakta dhatus are vitiated, which finally produce vataj, pittaj, and kaphaj granthi.

Clinical Features or Symptoms
According to Ayurveda, the clinical features of vataj granthi are:
- Pain when stretching
- Softness and distension, like the urinary bladder
- Sudden increase and decrease in the size of granthi
- Clean or bloody discharge in the case of rupture
- Infections
Management
- An ointment of pasted Himsra (Capparis spinosa), Rohini (Mallotus philippensis), Amruta (Tinospora cordifolia), Shayonak (Oroxylum indicum), Bilva (Aegle marmelos), krishnagandha (Moringa ovalifolia) with cow milk is applied to the granthi.
- Use of dashmool (10 herbs) with all oleaginous substances (oil, ghrita, fat, and marrow).
