Abstract

Hypertrophic osteodystrophy is a bone disease which commonly occurs in fast-growing large and giant breed dogs and sometimes occurs in medium-breed animals such as Australian shepherds. It usually affects the ability of the dog to move, it can be hereditary (present at birth) or can be due to injuries or nutritional deficiencies. Every disorder is a different problem which is found in the different parts of the body. The skeletal system provides a supporting framework with which muscles become firm and protect the softer tissues and bones are categorized according to shape- Long bones (typical bone which includes the femur and humerus), Flat bones (outer layer of compact bone which is spongy bone inside), Short Bones (There is no medullary cavity such as in carpal and tarsal bone), Irregular bone (seen in vertebrae). The size of the hindlimb of bone varies due to variations in the size of dogs. The femur is the heaviest and largest bone and Patella is the largest sesamoid bone of the body. Hypertrophic Osteodystrophy

Introduction

Hypertrophic osteodystrophy is an orthopedic development disorder which causes pain, swelling and limping and is seen in young rapidly growing breeds of dogs it usually affects the metaphyseal region of long bones or any bone can involve, but mostly affects the lower limbs. The distal ulna, radius and tibia are mostly affected radiographically seen in the metaphysis and get separated from the growth plate by an excessively dense band of bone. As this progresses there is deposition outside the periosteum and soft tissues get swollen up with subperiosteal hemorrhages which are affected around metaphyseal areas. The swelling is due to the fibrous thickening of the periosteum and the deposition of new periosteal bone seen between the age of 3 months to 6 months. It can be referred to as metaphyseal osteopathy. Sometimes it can also occur on the pet’s jaw, ribs and vertebrae. Growth plates where metaphysis is seen can grow cartilage which gets hardened during maturity which causes an issue in the flow of blood which can harden the bone properly, so when new bone continues to grow without proper hardening it causes inflammation which is the main symptom in dogs.

Pathophysiology

  • It is due to disturbances of the metaphyseal supply of blood which delays the ossification of the metaphyseal hypertrophic zone.
  • Inflammation and necrosis which is occurring secondary to metaphyseal trabecular microfractures.

Causes

  • It can be due to decreased uptake of vitamin C or excessive intake of other vitamins and minerals.
  • Infection with canine distemper virus (CDV).
  • Unbalanced diet for growing.
  • Familial clustering
  • Autoimmune nature of the disorder

Clinical Features

  • The primary characteristic is lameness
  • Swelling of the metaphysis of a long bone involves four limbs and is more seen in thoracic limbs.
  • In the acute form, there is transient low-grade pyrexia and swollen painful limbs.
  • Lethargy
  • Dehydration and severe palpitation with severe pain in the affected area.
  • Angular limb deformities.
  • Shaking of limbs and unwillingness to put the weight on the front legs.
  • Limb involvement is bilateral and involves distal ulna and radius which is episodic.
  • Nasal and ocular discharge.
  • Hyperkeratosis
  • Tooth enamel hypoplasia
  • Pneumonia
  • Reduced appetite

Treatment

  • NSAIDs to treat pain and give supportive care.
  • Correction of the inappropriate diet with the withdrawal of excessive supplements of Vitamin C, Vitamin D and various other minerals.
  • Intensive management with prednisolone, and nutritional supplements and provides various nutritional care.
  • Proper monitoring of animals.
  • In Angular deformity, there is surgical treatment.
