Introduction

Waldenstrom macroglobulinemia is a rare type of malignant blood cell cancer that begins by excess production of abnormal white blood cells in the bone marrow that crowd out healthy blood cells. This disease is a kind of non-Hodgkin’s lymphoma, which starts in white blood cells called Lymphocytes, it is a part of our immune system that acts as a defence barrier against microorganisms. Some of the lymphocytes are called B cells, and act as the body’s defence power. In this disease this process disturbs and the B cells turn into cancer cells, which means they grow out of control. Waldenstrom macroglobulinemia is characterised by the presence of elevated serum viscosity, high level of macroglobulin (immunoglobulin M [IgM] and the presence of a lymphoplasmacytic infiltrate in the bone marrow. Due to these factors blood becomes thicker than normal and makes it harder to move through blood vessels. Waldenstrom macroglobulinemia is a disease that occurs more frequently in older males because of its indolent nature, many patients are able to live actively and with treatment patients may experience many years of symptom-free remission.

Causes

  • Experts say that changes in DNA may lead to Waldenstrom macroglobulinemia. Most of the time, these affected cells have changes on genes called MYD88 and on CXCR4. The DNA changes may cause the genes to get stuck in the ‘on’ position, so cells live longer than normal. These changes mostly happen in later stages of life so most affected patients are of 50 or older people.
  • The disease is more common in men than women.
  • If someone already has a condition called MGUS (monoclonal gammopathy of undetermined significance) more reliable to get Waldenstrom macroglobulinemia.
  • White people are more likely to be effective than other races.
  • Genetic Factor: If any family member has already been diagnosed with Waldenstrom macroglobulinemia or any other type of B-cell lymphoma, then patients also have a highly increased risk of developing the disease.
  • Environmental Factors: Exposure to farming, wood dust, pesticides, and organic solvents.

Pathophysiology

Waldenstrom macroglobulinemia is a type of malignant cancer affecting two types of cells that are plasma cells and lymphoplasmacytoid cells. These both are WBCs. It is characterised by the presence of elevated serum viscosity, circulating antibodies, high level of macroglobulin (immunoglobulin M [IgM] and the presence of a lymphoplasmacytic infiltrate in the bone marrow which is made and secreted by the cells involved in the disease.

Waldenstrom macroglobulinemia grows and spreads slowly and is a type of lymphoproliferative disease which has similar clinical characteristics with the indolent non-Hodgkin lymphomas.

Sign And Symptoms

Waldenstrom macroglobulinemia usually begins in later stages of life and is a slow growing indolent cancer. Firstly appearing symptoms in these patients are weakness and extreme fatigue caused due to shortage of red blood cells. Increased level of IgM can thicken the blood viscosity and impairs the circulation leads to Hyperviscosity Syndrome. Patients show signs of anaemia, easy bruising, bleeding from nose and gums. Other symptoms are headache, fevers, night sweats, lack of appetite and weight loss without trying, frequent infections or allergies, breathlessness, changes in vision, numbness or tingling sensation in hands and feet, confusion, dizziness, clumsiness, peripheral neuropathies, enlargement of lymph nodes, hepatomegaly, splenomegaly, and rarely stroke or coma.
