Abstract

Introduction
Macular edema is a collection of a fluid and protein on or under the macula. It is a yellow central area of the retina and this collection of fluid and protein causes it to swell up and thickens. This macula is the inner layer of the retina which is centered on the foveola. This swelling may distort the person’s central vision, as this macula holds the Cones of the eyes that provide a sharp, clear, central view of the object. This accumulated fluid damaged the blood vessels in the nearby retina. This is a very sensitive area of the eye and generally causes vision loss or distorted vision.
Etiology/Causes
This disease is associated with a number of factors. Some of the common factors for this disease are
- Diabetes
- Postoperative complication following by cataract surgery
- Central retinal vein occlusion
- Age related macular degeneration
- Chronic uveitis
- Any blockage of a vein in the retina
- Side effects of the drugs such as latanoprost, epinephrine, rosiglitazone, adrenaline eye drops (especially for Aphakia glaucoma)
- Intraocular inflammations such as pars planitis, Behcet disease
- Some congenital diseases such as retinitis pigmentosa and retinoschisis
Clinical features/Signs and symptoms
The person who is suffering from macular edema may present following symptoms
- Primary symptom is blurry vision in the centre of the field of vision
- Distorted vision
- Photophobia
- Color may appear washed out or faded
Diagnosis
Diagnosis is made using following examination
- Ophthalmoscopy: This test clinically reveals a typical Honey-comb appearance of macula. This disease is also examined under the slit lamp.
- Fundus fluorescein angiography: This test demonstrates leakage and accumulation of fluid in the macular region.
Other tests are
- Visual acuity
- Dilated eye exam
- Optical coherence tomography
Complication
If this disease is left untreated then it may end into lamellar macular hole
Treatment
There is no permanent cure for this disease. We can manage this condition symptomatically. We can give NSAIDS or cortisone eye drops. Diabetic macular degeneration is managed by managing sugar level.
