Globus Pharyngis Treatment With Herbal Remedies

ABSTRACT

Globus Pharyngis is a non-painful sensation of a lump or a foreign body in the throat. It is a clinical condition that is long-lasting, difficult to treat and there is a high chance of recurrence. Nasolaryngoscopy is done to diagnose the patient. It is noted that Gastroesophageal Reflux Disease is considered as the major cause of Globus Pharyngis. The symptoms frequently improve with eating and are generally unaccompanied by dysphagia. The condition is equally prevalent in men and women. Empirical Therapy with a high dose of proton pump inhibitors is reasonable for patients with Globus Pharyngis.

Globus Pharyngis

INTRODUCTION

Globus Pharyngis is a painful sensation of a lump in the throat. Patients usually complain the lump is non-painful but often annoying. Hippocrates records the first known case of Globus sensation around 2500 years ago. In this condition, there is pressure on thyroid cartilage due to the contraction of the strap muscle of the neck. Identification and treatment are not always easy, as there is a range of possible causes. It can also be based on stress and anxiety so only a physical examination is not sufficient. Pathogenesis of Globus Pharyngis is unclear. Visceral hypersensitivity, mood disorders, and reflux diseases can be implicated.

CAUSES

Potential causes for Globus Pharyngis are:-

  • Gastroesophageal Reflux Disease
  • Abnormal Esophageal Upper Sphincter Dysfunction
  • Thyroid Dysfunction
  • Retroverted Epiglottis
  • Esophageal Motor Disorders
  • Upper Aerodigestive Malignancy
  • Rare Laryngopharyngeal Tumours
  • Hypertrophy of the base of the tongue
  • Psychological factors and stress
  • Tonsillitis and chronic sinusitis

SYMPTOMS

Certain symptoms and characteristics suggest Globus Pharyngis:-

  • Neck or throat pain
  • Weight loss
  • Abrupt appearance
  • Dysphagia
  • Regurgitation of food
  • Muscle weakness
  • Neck lump
  • Catarrh/ mucus that is unable to clear

DIAGNOSIS

Patients with typical Globus Symptoms usually require no further investigations. However, patients with alarming signs such as odynophagia, dysphagia, throat pain, and weight loss should undergo more extensive evaluations.

Barium Swallow

Barium Swallow studies have been reported to find benign lesions. This test seems to have limited diagnostic value in an investigation of patients with Globus Pharyngis.

Videofluoroscopy

Videofluoroscopy may help to identify pharyngeal dysfunction in a substantial proportion of Globus patients.

Flexible Esophagogastroscopy

Excellent views of the pyriform fossa and the postcricoid area can be achieved by insufflating air by flexible esophagogastroscopy.
