Abstract
Transverse myelitis is a neurological disorder. Neurological disorders are disorders that affect the brain and nerves found throughout the human body and the spinal cord. Introduction
Transverse myelitis is a disorder caused by the inflammation of the spinal cord. It is characterized as neurological dysfunction in motor and sensory tracts on both sides of the spinal cord. Transverse myelitis interrupts the messages sent by the spinal cord nerves throughout the body. It often damages the insulating material covering nerve cell fibers known as Myelin. This can lead to muscle weakness, pain, paralysis, altered sensation, and urinary and bowel dysfunction.
Causes
Most of transverse myelitis cases are Idiopathic – meaning the cause is unknown. However, the remaining cases are associated with Autoimmune disorders such as multiple sclerosis, systemic lupus erythematosus, neuromyelitis optica, Sjogren’s syndrome and sarcoidosis and others. Transverse myelitis is a condition striking all age groups regardless of family history ,gender and race. However some viral , bacterial and fungal infections affecting the spinal cord may also cause transverse myelitis. In most of the cases, the inflammatory disorder appears after the recovery of prior infection

Parasites and fungal infection rarely infect the spinal cord.

Symptoms
Sign and symptoms of transverse myelitis may usually develop over a few hours to a few days and sometimes may progress gradually over several weeks which include:
- Pain. Transverse myelitis pain may begin suddenly in your lower back .However Sharp pain may shoot down the legs or arms or around your chest or abdomen. It varies based on the part of the spinal cord that’s affected.
- Weakness in arms or legs. Patients feel heaviness in the legs, or that they’re stumbling or dragging one foot. Some may develop severe weakness or even total paralysis.
- Bladder and bowel problems. It includes frequent urination, urinary incontinence, difficulty in urinating and constipation.
- Abnormal sensations. Some patients with transverse myelitis may have tingling sensations , numbness, coldness or burning sensation. Some patients are sensitive to the light touch of clothing or to extreme heat or cold.
Diagnosis
Transverse myelitis can be diagnosed on the basis of signs and symptoms, medical history, a clinical assessment of nerve function, and test results. The tests which may indicate inflammation of the spinal cord and rule out other disorders, include the following:
- Magnetic resonance imaging (MRI)
- Lumbar puncture (spinal tap)-
- Blood tests
Some other blood tests can identify infections that may contribute to transverse myelitis, or rule out the other causes of symptoms.
Complications
The Most Common Complications Are
- Pain : It is one of the most common symptoms and a lasting complication too.
- Sexual problems : it is a common complication of transverse myelitis . Men may have trouble getting an erection. Both genders may have difficulty in reaching orgasm.
- Spasticity : There may be stiffness , tightness and painful spasms in the muscles most commonly in the muscles of buttocks and legs.
- Anxiety or depression : it is most common in patients with long term complications because of the various changes in lifestyle, stress and other factors.
- Paralysis: There may be partial or total paralysis of arms, legs or both.
