Crigler Najjar Syndrome Treatment with Herbal Remedies
Abstract
Crigler Najjar syndrome is also known as familial non hemolytic unconjugated hyperbilirubinemia or hereditary unconjugated hyperbilirubinemia. Crigler najjar syndrome is a genetic disorder related to the imbalance in the functions of the liver. It is an autosomal and recessive disease. There are raised levels of bilirubin observed in the plasma. Whenever there is damage or injury to the liver, the bilirubin level rises i.e. both conjugated and unconjugated bilirubin levels rise up. In the crigler najjar syndrome the unconjugated bilirubin levels in plasma are on the higher side. The crigler najjar syndrome is seen in two forms which are type-1 and type-2. The type-1 crigler najjar syndrome is a very serious condition while type-2 crigler najjar syndrome is still a less serious condition as compared to type-1.
Introduction
Crigler najjar syndrome is inherited from one generation to the other. In this syndrome, Uridine 5′-diphospho-glucuronosyltransferase (UDP-glucuronosyltransferase, UGT) levels fall to zero i.e. it is absent. This component is necessary for the proper metabolism of unconjugated bilirubin. When UDP-glucuronosyltransferase is absent, the levels of unconjugated bilirubin become very high. Hence, it hinders the overall health of the person. There are two types of crigler najjar syndrome depending on the levels of UDP-glucuronosyltransferase present. Type 1 is a very serious condition because in this case, UDP-glucuronosyltransferase is totally absent while in patients of type 2 crigler najjar syndrome, instead of pure absence, there are very low levels of UDP-glucuronosyltransferase available.
Causes
There is no specific known cause of the crigler najjar syndrome but it is believed to be due to the mutation of a gene. The genes play a very vital role in the production of proteins. When there is a mutation or change in the gene it will lead to the production of a defective protein which further hinders various natural mechanisms of the body.
Symptoms
The person suffering from crigler najjar syndrome presents various types of symptoms depending upon the condition of the body. Some of them are vomiting, fever, diarrhea, confusion etc. The person may experience difficulty in walking and falls while walking. He/she may experience seizures. Speech will also be hampered, they experience difficulty in speaking. Swallowing also becomes difficult. He/she may show changes in gait.
Diagnosis
The crigler najjar syndrome is hyperbilirubinemic condition so there are various laboratory tests that need to be performed such as: Liver function tests are done, etc. MRI reports are needed to monitor the condition of the heart.
Treatment
The contemporary system of medicine manages the crigler najjar syndrome by using phototherapy. Skin is exposed to light to cure the condition.
Ayurvedic Aspect
The condition of crigler najjar syndrome is majorly caused by a defect in the functioning of the liver. It can be correlated to the pitta imbalance and yakrit vikrati. It can be correlated to a condition caused by hindered equilibrium in the levels of rakta dhatu and pitta dosha . The sthan i.e. home of rakta dhatu and pitta dosha is believed to be the yakrita (which can be correlated to liver), hence abnormalities in the rakta and pitta will lead to abnormal functioning of yakrit and vice versa. Therefore to rectify the condition, it is important to restore the balance in the levels of pitta dosha, as well as strengthen the yakrita. There are various herbs that benefit the yakrit such as bhu nimb (kalmegh), kaasni, apamarg, aparajita etc.
