Abstract
Cervix is a tube-like passage that joins the uterus with vagina. Cervical polyps are the abnormal growths not cancerous (benign) commonly seen on the cervix where it adjoins the vagina, and can occur alone or in groups also. They usually vary in size and colour. These polyps are mostly cherry-red or reddish-purple or greyish-white. Usually these polyps are small, about 1-2 cm long. Polyps can be easily misdiagnosed with fibroids as they resemble imaging tests like USG and both have similar symptoms of abdominal cramps and abnormal menstrual bleeding. So to know whether it’s polyp or fibroid we should know about tissue structure and cells that are responsible for growth. Main responsible tissue is endometrial tissue that also lines the uterus and here forms the polyp. In this article we are going to discuss symptoms and the role of treatment by the healthcare team for Cervical polyps patients.
Introduction
Cervical polyps are abnormal benign or malignant growths that occur on the cervix. Malignant polyps seen for 0.2 to 1.5 % of the cases and more likely to be in postmenopausal women. These are of different sizes, shapes and origin, can be presented as alone or multiple, lobular or tubular shaped, is of cherry red and greyish white in colour, depending upon the vascularity of lesion. Its size also varies as mentioned earlier it can be typically less than 3 cm in diameter and can be even large enough to pack the vagina. Anatomically, a cervical polyp is connected to the surface by pedicle, which is also variable in size and length as these are usually long and thin but can be presented as broad and short based.
Etiological Factors
Causes are unknown in most of the cases. But many underlying theories have given by experts some of them are –
- Cervical polyps may result from the congestion of cervical blood vessels that leads in disruption of blood flow and ultimately results in polyp development.
- Chronic inflammation of the cervix can cause abnormal changes in the cells.
- Increase in oestrogen level may cause excessive growth of cervical tissue
- Endometrial hyperplasia can also cause the above condition.
Risk Factors
- Pre and post menopausal women.
- Older women.
- Multigravida that has a history of multiple NSVDs.
- Old patient of cervical polyps.
Cervical Polyps can be classified on their Origin
- Endocervical Polyps
- Ectocervical Polyps
- Endocervical Polyps – are the most common type of cervical polyps, usually occurs in premenopausal women located at the endocervix area and arises from cervical glands.
- Ectocervical Polyps – are more commonly occurs in postmenopausal women and arise from outer surface of cervix but located within the ectocervix.
Symptoms
About two of three women who have cervical polyps are asymptomatic. It can be diagnosed during abdominal or transvaginal ultrasound, during a pap test or during other routine gynaecological examinations.
Symptomatic women can present with
- Abnormal uterine bleeding like heavy menstrual bleeding
- Bleeding between two cycles
- Postmenopausal bleeding
- Vaginal discharge
- In very rare cases, Large polyps may block the cervical canal and lead to infertility.
- Bleeding after intercourse
