Introduction about Mouth Cancer
Mouth cancer is a type of cancer which can be in any part of the mouth or oral cavity. The parts are usually lips, tongue, lining of cheeks or gums. The cancer that occurs in the inner lining of the mouth or inside the mouth is called oral cancer.

- Benign Tumours (solid, cystic)
- Premalignant Lesions
- Malignant Lesions (carcinoma, nonsquamous malignant lesions)

1. Benign Tumours (solid, Cystic)
Solid:
- Papilloma: Papilloma are common in the oral cavity. Age group most prone to papilloma is in the third to fifth decade. Papilloma appear on soft, hard palate, uvula, tongue, lips. Their surface is irregular.
- Fibroma: It can occur anywhere in the oral or oropharyngeal mucosa. Usual cause is chronic irritation. They are smooth to firm in consistency.
- Haemangioma: Haemangioma mostly seen in children. Haemangiomas are basically of three types. Capillary, cavernous, mixed. Haemangioma are large and persistent.
- Lymphangioma: Lymphangioma mostly involved the part of the tongue. May involve tongue diffusely or may present as localized soft swelling which is compressible.
- Torus: Torus submucosal bony outgrowth. May involve a hard palate or mandible part of mouth. It may present as a narrow ridge or mass in the midline of the hard palate.
- Pyogenic Granuloma: Usually occurs in response to trauma or irritation. The location of mouth or oral cavity involved is anterior gingiva.
- Minor Salivary Gland Neoplasms: This is the commonest type. Site is soft or hard palate but can occur anywhere in the oral cavity. Presentation is a painless submucosal nodule.
- Solitary Fibrous Tumour: Most common locations are oral toungue nd buccal mucosa. It may occur in any other location like naso[pharynx, soft palate, salivary glands. It is a benign kind of tumour. It is painless, slow growing, well demarcated, mobile, submucosal tumour.
Cystic:
- Mucocele: Common site is the lower lip. It is a cyst of minor salivary glands. The lesion is a soft and cystic mass of bluish color.
- Ranula: It is a cystic lesion in the floor of the mouth on one side of the frenulum and pushing the tongue up.
2. Premalignant Lesions
Leukoplakia: Buccal mucosa and oral commissures are the sites involved. Floor of mouth, tongue, gingivobuccal sulcus and the mucosal surface of the lip. Buccal mucosa is the most common site. It is seen mostly in the fourth decade of life. Present with a smooth or wrinkled white patch. The chances of leukoplakia to become malignant is 1 to 17.5%.
3. Malignant Lesions
These are the cancerous lesions in the oral cavity
- Carcinoma Lips: It is squamous cell carcinoma. It is prevalent in males of the 40-70 age group. Lower lip involvement is more than upper lip involvement. Lesion is of ulcerative type. Metastasis may develop late. Site is between midline and commissure of the lip.
- Carcinoma Buccal Mucosa: It is the common type of carcinoma. Equally seen in males and females. Carcinoma of buccal mucosa is next only to tongue cancer.
- Carcinoma Oral Tongue: Involves anterior two thirds of tongue is common in males of age group 50-70 years. May develop pre-existing leukoplakia. Vast majority of such cancers are squamous cell types. Locally it may infiltrate deeply into the lingual muscle causing ankyloglossia or may spread to the floor of the muscle. Lym[ph node metastasis goes to the submandibular and upper jugular nodes and to the submental and jugulo-omohyoid.
- Carcinoma Hard Palate: It is common especially in people who have the habit of reverse smoking. Both men and women are affected. Cancer starts as a superficial ulcer with rolled out edges and gives no symptoms except painless irregularity on the palate felt by the tongue.
- Carcinoma Alveolar Edges: It is called gingival carcinoma. It is mostly seen in men. Lower jaw behind the first molar is the usual site of involvement. Tumour may spread to the cheek, floor or hard palate. It may invade the underlying bone and the neurovascular bundle.
Symptoms Of Mouth Cancer
Following are the common symptoms of the carcinoma in mouth:
- Sore in lip or mouth
- A whitish or reddish patch or plaque in the mouth
- Loose Teeth
- A growth or lump inside the mouth
- Pain in the mouth
- Pain in ear
- Painful swallowing
Causes/Risk Factors Of Mouth Cancer
The cancers in the mouth begin in squamous cells. Oral cancers are usually squamous cell carcinoma.
- Use of tobacco via cigar, bidi, hookah. any way of tobacco like hookah, cigar, chewing tobacco may lead to cancer of the oral cavity.
- Excessive drinking of alcohol
- Excessive exposure to the Sun
- Human papillomavirus (HPV) may cause cancer by sexual contact with infected person

Preventing Mouth Cancer
- Avoid excessive intake of alcohol or quit it as much as possible.

- Avoid exposure to excessive sunlight.

According to ashtanga in sannipaatika oshtha the lips become different colored and foul smell, thick fluid oozes out. The lips are usually inactive, swollen, and painful. It is called carcinoma of the lips.

Carcinoma of tongue can be compared with allas.
