Abstract
Cocoon syndrome is a rare disease in which thick peritoneal membrane wraps the intestine. It may be either primary or secondary to other causes such as previous abdominal surgery. It is mainly caused by the inflammatory process. Abdominal pain and intestinal obstruction are the main symptoms of cocoon syndrome. Introduction
Cocoon syndrome is a rare condition that causes small bowel obstruction. It is also called encapsulating peritoneal sclerosis (EPS) or sclerosing encapsulating peritonitis that leads to formation of a fibro-collagenous peritoneal membrane that involves the small intestinal loops. It has been reported in adolescent girls living in tropical regions. The first case of cocoon syndrome was described in 1978. The exact cause of this disease is unknown but the cases seen of cocoon syndrome have been limited to tropical and subtropical zones. This disease is the outcome of several different causes. The major causes that are responsible for cocoon syndrome are use of beta blockers, post-renal or liver transplant, tuberculosis and continuous ambulatory peritoneal dialysis etc. The main symptoms shown by this disease are abdominal pain and intestinal obstruction. In case of intestinal obstruction surgery can be performed. In most cases cocoon syndrome can be diagnosed by laparotomy easily. Let’s discuss cocoon syndrome in detail.

Diagnosis

- Laparotomy- it is a surgical incision into the abdomen that helps in diagnosis of any abdominal disorders.
Treatment
- Prevention of encapsulating peritoneal sclerosis (EPS)- Continuous ambulatory peritoneal dialysis is the most important risk factor which predisposes to occurrence of EPS in patients with chronic kidney diseases. Use of anti-fibrotic agents and tamoxifen helps in prevention of encapsulating peritoneal sclerosis.
- Correction of underlying cause
- Medical therapy-Medical therapy includes various types of pharmacological agents such as steroids, mTOR inhibitors and tamoxifen.
- Surgical treatment- in this procedure complete resection of membrane should be done.
