Management of Scapular Dyskinesis in Ayurveda
ABSTRACT
Musculoskeletal systems help to provide the shape of the body and also help in day-to-day activities. Due to injury, degenerative changes with age and diseased conditions lead to improper functioning of the body and also patients suffer from painful conditions. Scapular Dyskinesis is known as the loss of normal scapular movements. The reason usually remains an injury due to which loss of movements of the scapula occurs. The common Symptoms of Scapular dyskinesis are pain and loss of normal activity. In this article, we are going to discuss Scapular Dyskinesis and its ayurvedic management.
INTRODUCTION
Scapular rotation is important for the normal functioning of the shoulder. It can also be explained as an alteration in the normal motion, dynamic, and position of the scapula while coupled with scapulohumeral movements. Any bony substances or tissue around the shoulder can alter the roles of the scapula in motion or during the resting state. Most commonly the reason behind Scapular Dyskinesia is an injury or the abnormal repetitive use of the shoulder. In some cases, changes might be asymptomatic and increase the chances of injury. The numerous muscles that attach to the shoulder are commonly responsible for the complex motion and any deviation can cause damage to the cartilage, shoulder labrum, and rotator cuff.
Ayurvedic Point of View
In Ayurveda, Scapular Dyskinesis can be compared with Vata doshas vitiation by Abhi-Ghata (trauma). Vata doshas block the channels that further create more health issues. When patients indulge in Vata Vitiating Diet (Ahara and Vihara) then symptoms can be more severe and it causes various abnormalities such as Convulsion, tremors, pain, paralysis, facial paralysis, Scapular Dyskinesis, etc.
Symptoms
The Following Sign and Symptoms of Scapular Dyskinesis are
- Tenderness and pain around the scapula while using the arm over the head.
- Pain can get worse by lifting heavy objects.

- Asymmetrical posture or position (affected side usually goes down)

- Sports that involve overhead activities because in overhead athletes there is a 61% chance of developing Scapular Dyskinesis.
- Loose joints.
- Prior shoulder injuries.
- Poor strength and flexibility
- Kyphosis (abnormal curvature of the upper back)
Identification of Scapular Dyskinesis
- Scapular malposition.
- Coracoid malposition
- Pain
- Loss of scapular movements
- SICK posture
- Inferior medial border prominence
Diagnostic Method
1. Scapular Dyskinesis Test
A prominent medial border indicates a positive Dyskinesia test, this happens when continuous flexion of arm elevation with a maximum of three to five times produces the Dyskinesia. Scapular Dyskinesis is seen in the descent period of the arm motion.
2. Scapular Retraction Test
The test comes out to be positive if the pain rises by applying an external rotation motion. This test helps to know about the total strength of the rotator cuff in Scapular Dyskinesis. Active range of motion and pain is noticed.
3. Scapular Assistance Test
This test comes out to be positive if the pain is reduced or the range of motion is increased. In this test baseline, active range of motion and pain are assessed. And the therapist applies an assist to the scapular movements.
4. Isometric Scapular Pinch Test
In this test, the patient is asked to stand and squeeze the scapulae together as much as the patient can. Positive test reports show Scapular Dyskinesia with burning pain.
5. Wall Pushup Test
In this test, the patient is asked to perform wall pushups at least 15 to 20 times in a single interval.
