Abstract

Encapsulating Peritoneal Sclerosis or EPS is the rare cause of intestinal obstruction characterised by fibrotic encapsulation of bowel due to progressive intra- abdominal inflammatory process. Encapsulating peritoneal sclerosis is a clinical syndrome with the major signs of inflammatory parameters, gastrointestinal obstruction and macroscopic changes. By International Society for Peritoneal dialysis as a syndrome continuously, repeatedly or intermittently present with symptoms of the intestinal obstruction caused by adhesions of diffusely thickened peritoneum. Owtschinnikow firstly described about the encasement of intestines by fibrocollagenous membrane in 1907 and named the term as peritonitis chronica fibrosa incapsulata. EPS is also called as abdominal cocoon syndrome, obstructive peritonitis, calcific peritonitis, peritoneal fibrosis,peritoneal sclerosis and  sclerosing encapsulating peritonitis etc. Encapsulating  characterised by the presence of an inflammatory reaction and fibrosis in peritoneum which produces persistent, intermittent and recurrent symptoms of intestinal obstruction. Causes of Encapsulating Peritoneal Sclerosis

Causes of Encapsulating Peritoneal Sclerosis are not known clearly, but some of the causes are:

• Uremia
• Inflammation
• Loss of mesothelial fibrinolytic response
• Chronic exposure to biocompatible PD solutions

Diagnosis

• Physical Examination: Abdominal mass and abdominal distension is mainly seen in the physical examination.
• CT-scan: Different diagnosis are seen in CT- scan ie: Small bowel tethering, peritoneal calcifications, loculated ascites, dilated loops of bowel and thickened peritoneum.
• Plain radiograph: This test may show the gas-fluid levels similar to those in patients with any other cause of the small-bowel obstruction.

Wall of the cocoon may also calcify

• Ultrasound: In this test presence of ascites may be seen, clumped bowel loops and appearance of hyperechoic membrane are also seen.
• MRI: This will demonstrate the same features as CT scan, but MRI gives a better image of thickened bowel and peritoneal membrane as compared to CT scan.

Management of Encapsulating Peritoneal Sclerosis

In the management of EPS different therapies are used which includes:

• Nutrition support: Nutrition is very important in this condition and we have to firstly provide nutrition to the patient.
