Ayurvedic Treatment For Testicular Microlithiasis

ABSTRACT

Occurrence rate of microlithiasis has seemed quite prevalent from the past few decades. It could be due to various etiological factors such as improper dietary habits with an unhealthy lifestyle that promotes the formation of clusters of stones in the body. In this article our main topic of discussion is also a similar condition in which there is a formation of stones in the testicular region known as “testicular microlithiasis”. As a patient of this ailment might also suffer from other symptoms such as impotence, decreased sperm count etc. By using basic principles of dinacharya (ideal daily regime) we can attain sound health and that will help us in saving ourselves from other disorders. Testicular Microlithiasis

INTRODUCTION

Testicular microlithiasis is defined as the presence of stone in the testicular region. Seen only in males as it involves testicles they are the main part of male reproductive organs. In this small cluster of calcium or other substances got accumulated in the testicles. Testicles (two in number) are paired structures present in the scrotal sac just below the penis. It plays a major role in producing sperms through a process known as spermatogenesis. Within the testis numerous tubules known as seminiferous tubules are present that play a major role in sperm formation.

AYURDEVIC CORRELATION OF TESTICULAR MICROLITHIASIS

We can correlate it with “shukraj ashmari”. Shukra means semen and ashmari means stones. Presence of stones in the seminal discharge. Generally ashmari is of four types as per the root cause- vataj, pittaj, kaphaj and shukraj ashmari. In this there is a presence of vitiated dosha in the shukra vaha srotas (reproductive channel- testis and penis).

COMMON CAUSES

It includes all those common conditions that will give rise to generate a pathological condition in our body. As per ayurveda they are known as ‘nidan‘ and ‘hetu

  • Family history- Heredity also plays a major role in giving rise to this.
  • Excessive use of radiation therapy to treat other underlying ailments.
  • Environmental factors.
  • Use of unhygienic dietary habits.
  • Lack of physical fitness.
  • Presence of undescended testis (cryptorchidism).
  • Due to the presence of other infectious conditions such as- inguinal hernia, orchitis etc.
  • Carcinoma of testicular origin.
  • Infertility.
  • Intake of cold (sheet), madhur (sweet), snigdha (smooth) dietary food items in higher proportion.

PRODROMAL SYMPTOMS

They are the complaints that a patient feels before the appearance of signs & symptoms in the body. They are known as “purwarupa” according to the ayurveda.

As per the shloka- (Ashtanga hridaya. Nidan 9/8) following below listed prodromal symptoms are present:

  • Distension Of Bladder
  • Pain In Lower Abdomen
  • Passage Of Urine with goat’s urine like smell.
  • Fever
  • Anorexia
  • Pain in the bladder, testis and penis with a slight headache also.

SYMPTOMS

It means the general complaints along with which patients present in front of the physician.

  • Lump over the testicles.
  • Pain in the scrotal region.
  • Heaviness in the scrotum and the penile area.
  • Firmness and tenderness over the testicular area.
  • Lower back pain.
  • Low sperm count.
  • Itching over the scrotum.
  • Urinary Retention.
  • Difficulty in passing urine (Dysuria).
  • Fever
  • Fatigue And Malaise
  • Turbid Urine.
  • Blood in urine (Hematuria).

As per ayurveda: According to shloka (Ashtanga. hridaya. Nidana 9/9-10)
Following below listed symptoms are generally seen in this:

  • Pain in the lower abdomen and penis.
  • Stream of urine outflow will be fainted and rounded in appearance.
  • Haematuria (blood in urine).
  • Crystalluria (presence of cloudiness in the urine).
  • Turbid appearance of urine.

DIAGNOSIS

It includes the common diagnostic procedures we can use to recognize the disease.

  • Detailed history taken by using four diagnostic methods such as inspection, palpation, auscultation and percussion plays the utmost role in identifying the pathology.
  • By doing a detailed physical examination of the patient.
  • Urine examinations- urine culture test.
  • Routine blood tests- Complete blood count and blood sugar test.
  • Radiation examination:
    • X-ray
    • CT scan
    • Testicular ultrasonography

MANAGEMENT

Preventive measures:

  • Avoid consumption of processed and junk food.
  • Do not smoke.
  • Indulge yourself in regular exercises- perform mild exercises as per your strength and stamina.
  • Avoid severe unaccustomed exertion and harsh exercises after heavy meals or in cold weather.
  • Eat a healthy diet rich with good antioxidants and other important nutrients.
  • Increase the intake of liquid diet such as fruits enriched with water content and juices.

Sanshodhan Chikitsa (Detoxification Procedure)

As testicular microlithiasis involves the scrotal region of males, our acharya had already mentioned that over testicles, heart and eyes should not be fomented, or if necessary should be mildly fomented, similar to panchakarma procedures. So we need to use ‘mridu sanshodhan’ (mild detoxification procedures).

  • Snehana Karma (oleation therapy) – Smoothing the body dosha’s by using herbal ghrit/oil, apply it mildly over the scrotal area to smoothen the vitiated dosha.
  • Mridu Svedan (gentle fomentation) – Fomentation is done with herbs that possess mild action.
  • Mridu Vaman (Inducing emesis with mridu dravya (herb that are not very strong/harsh in action).
  • Uttar Basti – In this medicine in decoction form (kwath/oil) is put inside the body via Penis with the help of a specially made basti yantra (instrument). It will help in excreting out clusters of stones along with the medicines by urine.

Beside the above mentioned panchakarma procedure we also have another treatment modality known as san shaman chikitsa (alleviating treatment) in which medicines are given to the patient by giving medications to the patient through oral route.
