Abstract

Oromandibular dystonia is defined as closure of the jaw usually associated with teeth grinding and clutched jaws that is also a type of focal dystonia. It may also lead to the temporal-mandibular joint syndrome. In this dystonia type jaw, lower face and tongue are involved. Oromandibular dystonia can be of four types of jaw closure, jaw deviation, jaw opening and mixed form of oromandibular dystonia. In conservative treatment, the injection of botulinum is used as the first line of Treatment for Dystonia.

Introduction

Oromandibular dystonia is also known as cranial dystonia that is manifested by forceful contractions of muscles of the jaw, face, or tongue. This forceful contraction of muscles leads to difficulty in mouth opening and closing. Oromandibular dystonia also affects badly speech and chewing ability of a person. Cranial dystonia which is another term used for Oromandibular dystonia is a descriptive term used for dystonia that can affect any part of the mouth or face. Coming to single parts, Embouchure dystonia is the term used for the dystonia that affects the lips of the wind instruments playing musicians while Lingual dystonia is the one affecting the tongue. Women are more commonly affected by Oromandibular dystonia as compared to the men and its signs and symptoms start to appear in late stages of life, i.e. 40-50 years of age. Oromandibular dystonia is usually confused with Temporomandibular joint disease, however, the later is an arthritic disease condition. Signs and Symptoms

Oromandibular dystonia is particularly presented by the bunch of signs and symptoms:

  • Involuntary opening of the mouth
  • The jaw of the bottom shifts to the back or sideward
  • Forceful clenched shutting of jaw
  • Lips and face shows involuntary movements
  • The tongue has involuntary uncontrolled movements

Causes

The exact cause of Oromandibular dystonia is not known yet but in certain studies, it is concluded that this type of dystonia results from abnormally increased transmission of signals from the brain to the muscles of the face and jaw resulting in involuntary movements and closure of the jaw and face muscles. One more interesting fact about Oromandibular dystonia is that it is the only one neurological disorder to date which can be present with a family history or without it. Oromandibular dystonia can either be inherited from the family or can be acquired due to various secondary causes present in the environment like exposure to certain drugs or may be associated with other disorders like Wilson’s disease. The dystonia which gets due to drugs manifests initially by affecting the muscles of the face.

Diagnosis

Oromandibular diagnosis can be the one difficult to make at a first point due to various presentations of it, be it in forms or severities. No specific diagnostic test is yet found to diagnose this dystonia. Hence, its diagnosis is completely dependent upon clinical presentation, family history, neurological examination, and intramuscular electromyography.

Oromandibular dystonia is treated completely in an individualistic manner. In conservative treatment, several oral medications are given as per the signs and symptoms present.
