Arrhythmogenic Right Ventricular Cardiomyopathy (ARVC) Treatment in Ayurveda
Abstract
Lifestyle in today’s world has extremely changed from what it was some centuries ago. Due to globalisation and advancement of the techniques that have eased the life of a man has also given a blow to the health of an individual. Reduction in physical activities and faulty dietary habits have added to the problems of an ordinary person. As the food is contaminated with chemicals and pesticides and water gets contaminated from a number of contaminants, people are falling sick and further dietary habits with bad fats have given a sharp rise in cases of heart diseases. And stress and depression have also added to the number of heart patients. Heart is an important organ of our body that pumps blood throughout the body. Normally the heart pumps the blood at regular pace and time and in diseased condition this rhythm and pace is distorted which give rise to a number of discomforting conditions. Arrhythmogenic right ventricular cardiomyopathy (ARVC) is familial disorder characterised by the irregular beating of the right ventricle. In this section below, we shall see this condition in a little detail. And we shall also discuss its management in the contemporary system of medicine as well as its management in the traditional system of Ayurvedic life science.
Introduction
Arrhythmogenic right ventricular cardiomyopathy is an uncommon disorder that may cause ventricular tachycardia and sudden death in young and healthy persons. Arrhythmogenic right ventricular cardiomyopathy is caused by genetic defects of the parts of the heart muscles which are known as desmosomes, which links the cells together. These are composed of some proteins and some of these proteins can possess harmful mutations.
Causes
Genetic
It is a disorder inherited in an autosomal dominant pattern that expresses variation.
Exercise-Induced
Studies have found that long term sports activity is also a cause of Arrhythmogenic right ventricular cardiomyopathy. This might result from very high and intensive training.
Signs and Symptoms
- Arrhythmias include an abnormal timing and pattern of the heartbeat such as racing heartbeat, skipping heartbeat, fluttering sensation or palpitations.
- Premature ventricular contractions such as irregular and extra heartbeats that happen when electrical signals start in the lower chamber of the heart.
- Ventricular tachycardia is continuous and rapid heart beats which originate in the ventricle. This usually lasts only a few beats. It can cause nausea, weakness, lightheadedness, vomiting and a feeling of skipping the heart.
- Syncope is a sudden loss of consciousness that can occur in some cases.
- Sudden cardiac arrest is the first sign of Arrhythmogenic right ventricular cardiomyopathy in some patients when the heart stops beating. This can prove fatal if not treated timely.
Patients’ first symptoms rarely associate with right heart failure such as weakness, peripheral edema, ascites and arrhythmia symptoms.
Diagnosis
- Electrocardiogram
- Signal averaged electrocardiogram
- Exercise stress test
- Echocardiogram
- Twenty-four hour Holter monitor
- Cardiac magnetic resonance imaging
- Genetic testing
- Cardiac computed tomography
- Right ventricular angiogram
- Electrophysiology study
- Cardiac biopsy
Management
- Sotalol which is a beta blocker and antiarrhythmic agent.
- Amiodarone and metoprolol.
- Angiotensin converting enzyme inhibitors.
- Warfarin to prevent thrombus formation and pulmonary embolism.
- Catheter ablation is used to manage intractable ventricular tachycardia.
- Implantable cardioverter defibrillators are best in preventing sudden cardiac death.
- Heart transplant.
Ayurvedic Aspect and Management
- Ayurveda is a life science that deals with herbal, mineral and herbomineral remedies and also emphasises on the dietary habits. In Ayurveda, heart disorders are referred to as ‘hridroga’.
- Sadhak pitta, avlambaka kapha and vyana vayu stay in the cardiac area and are responsible for the normal functioning of the heart. Any kind of vitiation in the above mentioned dosha leads to the development of heart diseases or hridroga. Consumption of excessively hot and dry food, excessive work, stress, holding urges that should not be withheld, heavy food, excessive fasting, injuries and old age etc are some of the main causes of hridroga as mentioned in ayurvedic texts.
- As fainting and irregular heartbeat are the typical symptoms of Arrhythmogenic right ventricular cardiomyopathy which makes correlation with vataja and kaphaja hridroga quite clear. Hence the correlation of this condition can be well established with vataja and kaphaja hridroga.
- Management in Ayurveda depends on treating the dosha vitiation which eventually ends the associated symptoms of that disease.
- Nagarjun rasa, hridayarnava rasa, chintamani rasa, vishveshwar rasa, trinetra rasa, kalyanasundara rasa, akik bhasma, mukta pishti, makardhwaja, prabhakar vati, shankar vati, pippali churna, pushkarmool churna, haritakyadi churna, shunthi kwath, arjuna ghrita, baladya ghrita, chyawanprash, vasavaleha, shilajit rasayan, hridayeshwar rasa, panchskar churna, dashmoolarishta are some of the ayurvedic formulations that are beneficial in managing the heart conditions (hridroga) including Arrhythmogenic right ventricular cardiomyopathy.
- Padmasana, savasana, surya namaskara etc should be performed on a daily basis. It helps keep the heart healthy.
